Mesenteric Panniculitis Forming Tumor-Like Mass: Case Report

Mesenteric panniculitis is a benign disease characterized by the presence of chronic inflammation affecting the mesentery. The disease is often associated with abdominal trauma or surgery, vasculitis, granulomatous disease, malignancies and infection. It is rarely seen in the mesentery of the colon. Grossly it can mimic a malign neoplasm, leading to a misdiagnosis. A 43 years old female patient had admitted to Obstetrics and Gynecology outpatient clinics of our hospital with strong dysmenorrhea and menorrhagia. After a gynecological evaluation, total abdominal hysterectomy and bilateral salphyngo-oopherectomy operation was performed. During the operation mass lesions derived from the sigmoid colon were detected and a subtotal colectomy was also performed by the general surgery team in the same session. Macroscopic evaluation of the colectomy material revealed multiple, solid, irregular masses with diameters ranging between 1,2 to 15 centimeters. On microscopic evaluation, fat necrosis, areas of sclerosis, inflammatory cells and proliferation of myofibroblasts were observed. Mesenteric panniculitis is a rare entity. It can be misdiagnosed as a malignant lesion by clinicians and pathologists. Current case is a typical example for such a misdiagnosis due to multiple masses derived from the sigmoid colon which were detected during a gynecological surgery.
