The Effects of Tadalafil on The Partial Ureteral Obstruction

Objective: Previous studies in different experimental models have shown protective effects of phosphodiesterase-5 inhibitors for kidneys. In the present study, the possible renoprotective effect of tadalafil, a specific phosphodiesterase-5 inhibitor, was examined during partial ureter obstruction.

Material and Method: Male Sprague-Dawley rats were divided into 3 groups (n =6) as sham (S), unilateral partial obstruction (P) and tadalafil treated (10 mg/72 hours, intragastric) obstruction (PT). Two weeks after obstruction, creatinine measurements performed in serum and urine, urinary cystatin-c levels were measured and GFR values were calculated. Histological examination was performed to evaluate degenerative changes in the kidney. The results are given as mean±standard deviation. Statistical comparisons were performed by one-way ANOVA and Bonferroni test, with p <0.05 accepted as significant difference.

Results: The serum creatinine concentration increased in the P group (0.48±0.10 mg/dL; p <0.01) and the PT group (0.41±0.06 mg/dL; p <0.01) when compared to S group (0.30±0.03 mg/dL). The GFR value was 2.56±0.56 ml/min in the S group, and decreased to 1.40±0.25 ml/min (p <0.001) in the P group and 1.71±0.21 ml/min (p <0.01) in the PT group. Urinary cystatin-c/creatinine ratio in the S group was calculated as 0.96±0.23. This ratio increased in the P group (2.34±0.50; p <0.001) and the PT group (1.41±0.38; p <0.05) compared to the S group. The difference between the P and PT groups was statistically significant (p <0.05). Histological examination revealed the tubular and glomerular damage, interstitial fibrosis and inflamma-tion in the P group. In the PT group, the glomerular surface area was higher compared to the P group (p <0.05), with less tubular damage and fibrotic changes.

Conclusion: Present study showed that tadalafil treatment significantly reduced renal damage and improved proximal tubule function during unilat-eral ureteral obstruction. Considering the incidence of stone-linked ureter obstruction, it is thought that this effect of tadalafil may have clinical im-portance.
