Antibiotic Spacer Usage and Comparison of Culture Results in Orthopedic Infections

Objective: In our study, we aimed to investigate the antibiotic spacer (AS) application and culture results in patients undergoing surgical treatment with the diagnosis of infection.

Material and Method: Patients who had AS for infection between January 2017 and January 2019 were included in the study. The sociodemographic characteristics, laboratory results and culture results of the wound were recorded. The patients were divided into two groups according to the wound culture results.

Results: Of the 40 patients included in the study, 22 (55%) (GrupA) were not found to reproduction in the wound culture, while 18 (45%) (GrupB) showed reproduction in the wound culture. The mean age of patients was GroupA 64.78 ± 19.95 and GroupB 62.05 ± 17.55. Patients were examined in 4 groups as chronic osteomyelitis, knee replacement, hip replacement and trauma according to their diagnosis. When the groups with and without reproduction in the culture results are compared; There was no significant difference between the groups in gender, diagnosis, sedimentation, C-reactive protein, and WBC values (p ≥0.05). Culture-reproducing patients were gram-positive bacteria in 9, gram-negative bacteria in 5, mixed infection in 3, and fungus in 1 patient.

Conclusion: In more than half of the patient cultures applied AS, no reproduction was observed in the culture. It was observed that antibiotics for gram-positive factors were preferred most frequently. AS is an effective method to combat orthopedic infections. Antibiotic treatment, which is not selected according to the type of bacteria, can decrease the effectiveness.
