Comparison of Hematological and Clinical Results of Centrifugal and Roller Pumps: Prospective Randomized Clinical Study

Objective: We aim to compare the preoperative and postoperative effects of centrifugal and roller pump heads used in cardiac surgery.

Material and Method: A total of 30 patients who underwent cardiopulmonary bypass with the bicaval cannulation method were included in the study.The patients were randomized into two groups as centrifugal and roller pumps. Preoperative and postoperative hemogram and blood gas samples, urine outputs and total drain output after cardiopulmonary bypass and in the postoperative 24th hours, ventilation time, and length of intensive care unit and hospital stay and blood products were compared for both groups.

Results: There was no difference between the groups in terms of mean age, body surface area and operative data (p >0.05). Mortality was not observed. There was no difference between groups in terms of mortality, hospital stay, intensive care unit stay and extubation time (p >0.05). It was observed that intraoperative lactate values were lower in the centrifugal group, but there was no statistically significant difference except the lactate values (p =0.048) taken at the 5th minute after cross clamp. There was a significant difference between the two groups in erythrocyte values at the 2nd postoperative hour (p =0.009).

Conclusion: No significant difference was found in terms of cardiopulmonary bypass period, postoperative period, mortality and morbidity regarding the effects of two pump heads. We believe that centrifugal pump heads can be used safely like roller pump heads. However, it is an important detail that the centrifugal pump is difficult to obtain, requires extra cost and the roller pump is integrated into the system and does not require additional costs.
