Objective: The aim of this study is to evaluate the imaging findings and accompanying malformations of pediatric patients with intracranial lipomas detected in computed tomography (CT) and magnetic resonance imaging (MRI) examinations and to compare pericallosal and cisternal lipomas radiologically.

Material and Method: Cerebral CT and MRI findings of pediatric patients with intracranial lipomas between January 2013 and March 2020 were examined. Locations, sizes, CT densities, shapes, presence of calcification, other imaging findings and accompanying anomalies of lipomas were evaluated.

Results: Lipoma in a single localization was observed in 16 out of 17 children (11 boys and 6 girls, between ages 2 and 16) with intracranial lipoma included in the study. Two lipomas at different localizations were detected in 1 patient, while 6 lipomas were pericallosal and 11 lipomas were located cisternally. The size of pericallosal lipomas (median 26 mm) was statistically significantly larger than cisternal lipomas (median 8 mm) (p =0.020). The shape of the cisternal lipomas was statistically significantly tubule-nodular (p =0.010). There was no statistically significant difference between the two groups with regard to age (p =0.733), gender (p =0.901), presence of calcification (p =0.653) and accompanying anomalies (p =0.057). Headache was the most common symptom of the patients.

Conclusion: Lipomas are rare congenital intracranial malformations. Early diagnosis of intracranial lipomas is important for neurological development of pediatric patients. It is extremely valuable to know radiological findings in terms of patient follow-up and management.
