Retrospective Investigation of the Effects of Low-Dose Acetylsalicylic Acid and/or Other Non-Steroidal Anti-Inflammatory Drugs on the Terminal Ileum and Colon Mucosa

Objective: In this study, it was aimed to demonstrate effects of low-dose ASA and/or other NSAIDs on terminal ileum and colonic mucosa.

Material and Method: In our study, data from 105 patients (aged>50 years) who underwent screening colonoscopy due to chronic constipation, chronic diarrhea, history of polyposis, blood in the stool or other reasons at the Endoscopy Unit of Gastroenterology Department of University Hospital between July, 2018 and November, 2019 were retrospectively reviewed.

Results: In the study, there were 105 subjects (53 men; age range: 27-86 years). No significant difference was seen in colonic lesions between patients with or without ASA and/or NSAIDs use; however, incidence of aphthous ulcers in terminal ileum was significantly increased in patients with ASA and/or NSAID s use (p =0.043). In regression analysis regarding biopsy results, male gender was associated with higher likelihood for negative result (p =0.026). In regression analysis regarding chronic diarrhea, it was found that likelihood of diarrhea was decreased by decreasing age (p <0.001) and that ASA and/or NSAIDs use decreased likelihood of diarrhea (p =0.03).

Conclusion: ASA and/or NSAIDs use induced enteropathy or colonopathy may be as severe and frequent as upper GI system complications. Symptoms and findings are non-specific in this entity with unclear pathogenesis. This leads delays in diagnosis and treatment. However, techniques targeting lower GI system such as capsule endoscopy, dual-balloon endoscopy have facilitated diagnosis.
